So the time came to install my new walnut gear knob, but first I had to remove the original leather one. I researched the many postings on removal and was prepared to protect my face and use brute force to pull it out - but despite several attempts I had no success. Given I had earlier injured myself doing something else I decided to take the safe (but longer) path and set up a simple lever and fulcrum. For the lever I used a piece of timber (about 3 * 1 inch * 3 feet) and cut out a rounded hole at one end so the lever would be able fit around the base of the knob. I then set up some bricks and wood as the fulcrum, inserted the wooden lever under the knob (Ideally a piece of cloth should be used to protect the knob) and was able to use very little force on the other end of the wooden level and the knob just came out very easily. If you have the time and inclination I would recommend the method, no doubt it could be further refined. 2 photos provided - bright sunlight but still clear enough.
