Get a block test kit and test the coolant for exhaust gasses.

When you top up the coolant after a 'loss event' is there any (at all) oil sheen on the top of the coolant in the header tank?

Is the oil level always rising when a coolant loss occurs?

Exactly which gaskets have been changed?

Did you test the original oil cooler, once it was off the car?

Are you absolutely sure to be following the correct coolant fill and bleed procedure. I get the feeling that the diesel is the easiest engine to do this on, but mistakes can still be made. An air-lock in the heater matrix is easy to create. make sure the car is on level ground (from my knowledge of the system I'd suggest that if on any sort of slope then make the front the higher end) when bleeding the cooling system - and don't rush filling it.

Last question: When removing the header tank cap with a cold or nearly cold engine is there any pressure release?
