I'm sure between the two o rings there should be a really tiny spacer but.. I maybe missing this piece and I would sooner have this spacer in place despite it appearing very tight and secure at present time.

I'm afraid you've lost the small spacer.
It would be worth scouring eBay for a similar sized one in nylon. Measure the pipe and the rail bore and maybe find something here:
