Thank you. So I think this is unignited fuel, like you get with a cold start before the engine fires up. I wish I could help, but I think it's gone beyond the point where it can be diagnosed over the Internet. Reading your opening post, you seem to have done everything reasonably possible to fix it.

It's weird that it is intermittent. All I can think of is that you have a loss of compression on one of the cylinders. Perhaps there is a sticking valve, or perhaps the compression is low enough to be on the edge of firing/misfiring.

Did you get chance to follow Mike's suggestion and get a T4 test of the compressions and fuel pressures? That is the obvious next step.
