Judgement day for me then.

Will this nerve block work, and if it does, how long for?

They done an anesthesia ilioinguinal nerve block with ultrasound guidance, no cutting of the nerves as yet, that will be discussed at Southampton hospital next month (microsurgical cord denervation).

Its now been 5 hours since the block, and I still have the pain. The surgeon says it can take days for the block to work, he says if it doesn't work, he has some other ideas in mind... I'm due to go back for a follow-up appointment, so lets see what happens, but I don't reckon this block has worked.

I learnt today that chronic testicle pain is now becoming very common, and is seen as a medical discharge from the army.

I emailed the GI unit and got a reply from the consultant, firstly apologising for the cockup with the appointment and a second apology for my next appointment being four months away, offered my a slot in a couple of weeks instead. I queried the wording he used, "not appropriate" to wait that long, kinda set alarm bells off in my head despite his reassurance he meant nothing by it.

Well! I seem to have jumped the gun a little, the nerve block has in fact worked. It seems to have brought some small amount of pain relief, Woohoo! its absolutely blissful. is this what normal feels like? as I've forgot.

I know the surgeon did say it can take a few days for the block to work, the steroids need time to take effect, but I gave up after the first 24 hours. It was in fact the eighth day that I had noticed a change, blooming amazing... Now, how long will it last?

Its three months until my next follow-up with the pain clinic, but at least I'm seeing another urology specialist at the end of this month, I hear he is meant to be one of the best...

So... has anyone got any words of encouragement?

I'm seeing this new urologist tomorrow at Winchester hospital, it was originally meant to be at Southampton, but they changed the date, and the location.

... again I'm worried.

80 mile round trip to just see a urologist.

Hi Dallas ive just caught up with all of this after a long time away. Seeing what you have been through the trip to see the urologist will be the easy. Best of luck for tomorrow mate,keep us posted.

Anyone can sell hope/encouragement and in reality
Wes, by now you are very experienced at seeing different specialists. Write some headers down about what you want to say and ask, then take it from there. It’s great news the injection worked and you are getting pain relief.

Thanks Phil, I've gained a lot of knowledge over the years researching Orchialgia, but I have to be careful as many of these specialists really don't like a wise-ass. You don't get anywhere when you go in sounding like a doctor, I've experienced this before.

Its certainly a strange situation to be in, one half of me wants them to do something, but for them to do something will most probably mean an operation, and who wants an operation down in that area...

I don't mind the examinations, but to go under the knife is another matter, the nerve block was bad enough. I think once I get myself in the zone I'll be fine, and a bit of encouragement from you guys obviously helps.

One thing you could ask is “Is the surgery reversible”, that is if it is offered during your appointment.

The pain from the shingles in the eye has not completely cleared up. You know that one Wes - neuropathic pain from the damaged Trigeminal nerve. I have a first appointment on Monday for a review and I am not expecting anything spectacular from it

I suppose if this urologist offers me surgery, it will depend on what surgery they offer. Microsurgical cord denervation being one option, but the surgery is not reversible. Basically the main blood and lymphatic supply of the testicle are preserved, and the remaining structures including the nerves are cut, but sometimes some nerves are missed.

The other options would be a varicocele embolization or removal of the epididymal cyst, but I get told these procedures have a high failure rate. This guy I'm seeing is meant to be one of the best urologist's in the UK, I just hope he offers some further investigation to help get a better diagnoses, rather than all this guest work.

As for your shringles Phil, I can imagine how awful that must be for you, do you have pain on all three branches of the trigeminal nerve, or just the upper two. I bet you must be on the strong stuff by now, I hope the drugs don't impact on your life too much, the medications alone can be life limiting.
